So in my example Player A is currently 15, however due to his year of birth should be playing in the U16's.
I am highlighting that a simple question when the player first registerd at another club is probably the real issue here. Could also be that someone made some error in that registration process and it just wasnt picked up until recently.
I think this paints a completely different picture to some posters suggesting there has been delibate cheating by a ceratin club.

Its one or the other. Its either deliberate or its a mistake.
Innocent errors are mistakes without the pre meditated intention to decive or cheat.
A deliberate mistake it deliberate full stop. The outcome is that you are intending to cheat or deceive from the start.

Clubs are run by volunteers and these mistakes do happen from time to time. However I guess this sort of 'club error' in incorrectly registering a player's date of birth is something that the FFSA expects 'self registration' will correct. After all if the player is being registered on-line by their parent, then there can be no mistake about the year their child was born.
Unfortunately, this is an idealist view, because so many new players being registered are coming from non-English speaking backgrounds and parents can hardly be expected to navigate their way through the My Football website to register their child to play football if they don't even speak the language.
But every year there is controversy with Man United Cup and FFSA JPL Cup finals and the fielding of ineligible players. The rules are for all to see on the FFSA website, and the FFSA always distribute to clubs well before these competition begins.
There should be very clear warnings given to clubs who have teams in the finals - play an ineligible player then be prepared to bear the consequences of a fine and disqualification. No excuses. Officials should check and double check their playing lists. It's as simple as that.
